Terms and Conditions
Welcome to State of Infusion. By accessing our website or using any services provided by State of Infusion, you agree to the following Terms and Conditions.
Eligibility
To use our services, you must be at least 18 years of age or be a minor receiving services with the consent and presence of a parent or legal guardian, and have the legal capacity to enter into a binding agreement.
Services
State of Infusion provides wellness services that may include intravenous (IV) therapies, injectable therapies, peptides, biologics, hormone-related services, nutritional support, and other wellness offerings. All services are provided under appropriate medical oversight and are individualized based on client needs, eligibility, and clinical judgment.
Appointment Booking
Appointments may be scheduled through our online booking system or by contacting us directly. You are responsible for providing accurate, current, and complete information when scheduling services.
Payment
Payment is due at the time services are rendered for in-office or on-site treatments. Payment for peptides, biologics, or other custom-ordered therapies is required in full prior to ordering and is non-refundable once the order has been placed. We accept cash and major credit cards. By providing payment information, you authorize State of Infusion to charge the full amount of the selected service(s).
Cancellation Policy
Appointments must be canceled or rescheduled at least 24 hours prior to the scheduled time. Cancellations made within 24 hours of the appointment may result in a cancellation fee equal to 50% of the total scheduled service cost.
Medical History and Informed Consent
Prior to receiving services, clients are required to complete a medical history intake and sign informed consent documentation. A Good Faith medical evaluation will be conducted by State of Infusion’s Medical Director or a delegated provider, which may occur by phone or secure video (Zoom/telehealth), as required. You agree that all information provided is accurate and complete and agree to promptly inform State of Infusion of any changes to your medical condition.
Medical Disclaimer
State of Infusion’s services are intended for wellness and supportive purposes only. Certain therapies may be investigational, used off-label, or not FDA-approved for all indications. Services are not intended to diagnose, treat, cure, or prevent any disease or medical condition. Individual results may vary. Clients should consult their personal healthcare provider regarding any medical conditions or concerns prior to receiving services.
Assumption of Risk and Limitation of Liability
By receiving services from State of Infusion, you acknowledge and accept the inherent risks associated with wellness services, including IV, injectable, peptide, hormone, and biologic therapies. To the fullest extent permitted by law, State of Infusion, its owners, medical providers, employees, agents, and affiliates shall not be liable for any direct, indirect, incidental, or consequential damages arising from or related to services rendered. You agree to release and hold harmless State of Infusion from any claims, damages, or liabilities related to your participation.
Governing Law
These Terms and Conditions shall be governed by and construed in accordance with the laws of the State of Texas.
Modifications
State of Infusion reserves the right to modify these Terms and Conditions at any time. Continued use of the website or services following any changes constitutes acceptance of the revised terms.
